How We Work
CURA offer a Person-Centred approach, which utilises the Young Persons Recovery Star to measure and review Outcomes agreed in partnership with Staff, Young People and their Families.
We accept referrals from Youth Offending Teams (YOT), Leaving Care Teams, Adolescent Mental Health and Forensic Services. We will request detailed key information regarding each referral prior to undertaking an assessment.
Referrals will be discussed weekly in our Multi-Disciplinary Referral Meetings where a decision will be made regarding which team members would be most suitable to undertake the assessment. We aim to decide on suitability of placement within two weeks of receiving the referral.
Prior to undertaking an assessment, Young People will be contacted to advise them of the date and time of the assessment and to obtain contact details of any family members/carers or significant others they may wish to be involved in the assessment process.
Detailed Assessments are completed by at least two Team Members, focussing on the following eight key areas as identified in The Young Persons Star:
- Semi-Independent and Supported Living
- Accommodation
- Work and Learning
- People and Support
- Health
- How you feel
- Choices and Behaviour
- Money and Rent
- Practical Life Skills
A further meeting will be arranged with the young person and Family/Carer or Significant other to agree an Interim Support Plan to facilitate the transition into our service, at this point a Key Worker will be identified. A one-page profile will be completed with each Young Person and carer within the first four weeks of their stay, identifying:
- What is important to the individual.
- What is important for the individual (i.e. What does good support look like?)
- What people like and admire about the individual.
Each of our Young People will develop and continuously review their own Person Centred Team Plan. Once a Young Person is offered a placement, they will then become part of that core team and the plan will be shared with them, which includes one page profiles already developed by staff members.
The Young Person will be expected to engage with their Key Workers on a weekly basis to complete further assessments including detailed risk assessments and to regularly review and update their individual support plans.
Each Young Person will be supported to develop their own weekly planner, which will include educational activities, social & recreational activities, activities of daily living, physical health & wellbeing and Key Worker Sessions.
CURA aim to work in partnership with our Educational Counsellor to provide a flexible and creative approach towards meeting national guidelines for provision of education, building on individual strengths & talents, linking in with the Young Persons identified targets and goals.
